PAYROLL DEDUCTION AUTHORIZATION FORM

This Form is for current Cape Fear Valley Employees to secure tickets to Crown Complex events

and deduct the cost from the employee's paycheck.

Payroll Deduction Ticket Availability

Only Crown Complex discounted events can be payroll deductable!!

​*Tickets will be emailed to the address provided on this form. You are responsible for

displaying your tickets on a smartphone to enter an event. Please allow up to 7 days for your order to be processed.

*You may pick any upcoming check to deduct from, but it must be before the event date.

Tickets purchased using Payroll Deduction will be ​emailed to the address provided on this form. You are responsible for displaying your tickets on a smartphone to gain entry to an event.

Please allow up to 7 days for your order to be processed. If you have not received your tickets via email after seven days, please get in touch with us using the information below.

Tickets can be split up into multiple payments, but all payments must be fulfilled before the event date.
